Specific dollar amount requested.

Identification of a contact person with telephone number, FAX number, and e-mail
address.

Board minutes showing action authorizing the district/AEA administration to make
the request to the SBRC.

Application form or district/AEA developed documentation supporting the request
not to exceed five pages.
